Kindergarten - 1st Grade
- 4th Dose of Tetanus, Diptheria and Acellular Pertussis (DTap)
- 3rd Dose of Inactivated Polio Virus (IPV)
- 2nd Dose of Measles, Mumps and Rubella (MMR)
- 2nd Dose of Varicella (VAR)
2nd - 5th Grade
- Proof of 2 doses of Measles, Mumps Rubella and Varicella
6th - 8th Grade
- Dose of Adult Tetanus, Diptheria and Acellular Pertussis (Tdap)
- Proof of 3rd dose of Hepatitis B
- Proof of 2 doses of MMR
- Dose of Meningococcal Vaccine (applies to 6th grade only)
- Second dose of Meningococcal (applies to 12th grade only)
